Job Summary

Resides in Prisma Health Clinical Integration and has responsibility to Management of organizations in partnership with Prisma Health. Participates in the planning, development and implementation of activities related to wellness promotion and to modification/prevention of behaviors and lifestyle choices that are associated with chronic disease. Will work in multiple locations such as corporations, the ECC, all Prisma Health campuses, and community partnerships as part of the Prisma Health Business Health program.

Accountabilities

• Participate in the creation and delivery of educational offerings and tools that assist with achievement of goals and objectives for community-based health promotion and disease prevention. Educational offerings will target populations of varying ages and may include group and individual sessions.Will serve as a clinical liaison to disease management coaches. - 20%
• Assist with major health initiatives or projects such as corporate flu screenings or on boarding of large acquisitions. Duties may include coordination of supplies, staffing to assist with project, documentation, education of other employees or patients, or development.revision of policies and procedures associated with such projects. - 15%
• Participate in the development of written procedures and tools for new or changing activities to support standards of performance, evidence-based practice and workforce education. - 15%
• Participate in the creation and use of reporting tools which includes, but not limited to, volume statistics. - 10%
• Participate in Business Health programs such as health fairs, screenings for local industries, education sessions, as assigned. - 15%
• Orient, precept and mentor new Wellness Nurses, as assigned. - 10%
• Participate in other clinically-based health and wellness activities, as assigned. - 15%

Supervisory/Management Responsibilities

• N/A

Minimum Requirements

• Associate's Degree in Nursing
• 2 years of experience in

  1) Direct care of patients with chronic medical conditions, particularly diabetes and cardiovascular disease; AND/OR

  2) Education - individual and group AND/OR

  3) Community-based healthcare AND/OR

  4) Care/Case Management or knowledge regarding community resources

Required Certifications/Registrations/Licenses

• Current SC RN license

Other Required Sills and Experience

• Basic Life Support (BLS) Certication, current - REQUIRED
• Knowledge for compliance with protected health information policies - PREFERRED
• Health education/disease management - PREFERRED
